Our Hospice at Home health care assistants and nurses aim to give patients that choice by working with other healthcare professionals to provide comprehensive care for you and your family. Your GP and the District Nurse will remain as your key contacts, and they, along with other health and social care professionals, will work together to help in carrying out a care plan to suit your loved one’s needs.
We provide care based on needs, which may include frequent visits during the day from one or two of our team and care overnight, if needed. We aim to manage symptoms such as pain, breathlessness and nausea, and to give your family support. As a family member or carer you may also benefit from the chance to have a rest, take a break or spend some relaxed time with your loved one. Our team of skilled and experienced health care assistants and registered nurses operate 24 hours, 365 days of the year and all care is provided free-of-charge.
