Salary - fte £29,896 pa (plus variable shift rates and on call allowance)

Hours – c 20 hours per week plus on call

Are you a registered nurse who wants to make a difference to patients and their families? At Longfield we care for adults in the last weeks of life who wish to be cared for and die in their own home. Our Hospice at Home team are looking to recruit an experienced registered nurse to work as part of the small team. This is a part time post working on a four-week rota that includes weekends, bank holidays and some out of hours on call. 

As a Hospice at Home Clinical Coordinator, you will be responsible for managing referrals, supporting staff, providing hands on care, ensuring the service runs smoothly and is responsive to the needs of patients. You will be working in partnership with GP’s, District Nurses and Palliative Care Teams to ensure seamless and coordinated care. We pride ourselves on the delivery of high quality person centred care, which is professional and supportive.

Do you have the skills we are looking for?

  • An RGN with experience in palliative / end of life care
  • Experience of caring for patients in a community / care home / hospital setting
  • Experience of caring for patients with a range of life-limiting illnesses
  • IT literacy and experience of electronic patient records
  • Experience of managing clinical staff
  • Mentorship / teaching experience is desirable

 

The position is subject to an enhanced Disclosure Barring Service (DBS) check, Occupational Health disclosure and references. 

Hospice at Home serves part of the county of Gloucestershire and visits can be anywhere within this area to meet the needs of our patients. Travel is an essential component of the role, therefore you will need to be a driver and car owner.
