We are looking for professional, empathetic and considerate staff who have experience in caring for patients with a range of life-limiting illnesses in a community or hospital setting. Longfield’s Hospice at Home team provides care to adults with any life-limiting illness whose choice is to be supported at home in the last few weeks of life, which includes support to their immediate families.
As a Hospice at Home Health Care Assistant you will be responsible for helping us to support patients through hands-on direct care, working in partnership with GP’s, District Nurses and Palliative Care Teams.Read more
You will be required to help maintain, configure and develop the use and functionality of our confidential patient records system. You’ll also need to run and format a range of reports, use your skills to help with clinical audits and be the first point of contact both for system users and for the external system provider, troubleshooting and solving problems as they arise.Read more
