This study day is aimed at those health and social care staff who support people wishing to plan for their future care.  It is especially useful if you are involved in palliative and end of life discussions.  The day explores the principles of ACP and the use of the Gloucestershire document ‘Planning for Your Future Care’.  It focuses on the communication skills needed for ACP incorporating some practical sessions.

Learning Outcomes:

  • Explain the key principles of ACP
  • Identify the opportunities and challenges involved with communication in ACP
  • Discuss the skills required to complete an ACP
  • How to introduce the ACP document to clients and families sensitively
  • Have increased confidence in discussing ACP
